სარეგისტრაციო პირობები და საინფორმაციო წერილი

საქართველოს ინტერვენციულ კარდიოლოგთა ასოციაციის (GAIC) IV საერთაშორისო კონგრესი

რეგისტრაციის გავლისას თქვენ ავტომატურად ეთანხმებით ქვემოთ მოყვანილ წესებსა და პირობებს:

1. ძირითადი სარეგისტრაციო წესები და დასწრება

  • აუცილებელი წინაპირობა: კონგრესზე დასწრებისთვის სავალდებულოა წინასწარი ელექტრონული რეგისტრაციის გავლა. კონგრესზე დამსწრეთა რაოდენობა შეზღუდულია და მოიცავს მაქსიმუმ 350 მსმენელს.

  • ადგილზე რეგისტრაცია: კონგრესის მიმდინარეობისას, ადგილზე რეგისტრაცია და მონაწილეთა აღრიცხვა იწარმოებს 26 ივნისს, 12:00-დან 13:00 საათამდე, ასევე 27 ივნისს.

  • გამოუცხადებლობა: კონგრესზე გამოუცხადებლობის ან არ დასწრების შემთხვევაში, კრედიტქულები არ გაიცემა, ხოლო გადახდილი თანხა დაბრუნებას არ ექვემდებარება.

  • სამიზნე აუდიტორია: კონგრესზე დასწრება განკუთვნილია კარდიოლოგების, ინტერვენციული კარდიოლოგების, შესაბამისი პროფილის რეზიდენტებისა და დაინტერესებული სამედიცინო ფაკულტეტის სტუდენტებისთვის. თუ არ მიეკუთვნებით აღნიშნულ კატეგორიებს, რეგისტრაციამდე გთხოვთ დაუკავშირდეთ საორგანიზაციო კომიტეტს (ტელ: 599 50 71 87, ელ. ფოსტა: georgiapcr2018@gmail.com).

2. კრედიტქულები (10-12 CME) და სერტიფიკატი კონგრესზე გათვალისწინებულია 2 ტიპის რეგისტრაცია. კრედიტქულების/პროფესიული და არაკრედიქტულებით, კრედიტქულების/პროფესიული სერტიფიკატის მისაღებად მოქმედებს შემდეგი წესები:

  • სწორი მონაცემები: რეგისტრაციისას უმნიშვნელოვანესია ელ. ფოსტის მისამართისა და პირადი ნომრის ზუსტად მითითება. კრედიტქულების მოთხოვნის შემთხვევაში, ყველა ველი უნდა შეივსოს უშეცდომოდ.

  • სავალდებულო დასწრება: პროფესიული რეგისტრაციისას/კრედიტქულების მისაღებად აუცილებელია კონგრესს დაესწროთ ორივე დღეს.

  • შეფასების ფორმა: სერტიფიკატისა და კრედიტქულების მისაღებად სავალდებულოა უკუკავშირის (შეფასების) კითხვარის შევსება, რომელსაც მიიღებთ ელ. ფოსტაზე ან შეავსებთ ადგილზე ბარ-კოდის დასკანერებით.

  • სერტიფიკატები გაიცემა მხოლოდ ელექტრონული ფორმატით, რეგისტრაციისას თქვენ მიერ მითითებულ ელ. ფოსტაზე.

  • ვის ენიჭება კრედიტქულა: CME ქულების მოპოვება შეუძლიათ მხოლოდ: კარდიოლოგებს, კარდიოქირურგებს, ინტერვენციულ კარდიოლოგებს ან შესაბამისი სუბ-სპეციალობის რეზიდენტებს.

3. საორგანიზაციო დეტალები

  • ლანჩი: კვების სივრცე განთავსებული იქნება მესამე სართულზე, ლანჩ-სიმპოზიუმების ოთახებში. მსმენელები უზრუნველყოფილნი იქნებიან ლანჩბოქსებით, რომლის არჩევაც შესაძლებელია იქვე ადგილზე რეგისტრაციის პროცესში.

  • ფოტო/ვიდეო გადაღება: კონგრესის მიმდინარეობისას იწარმოებს ფოტო და ვიდეოგადაღება. შესაბამისად, შესაძლოა უნებლიეთ მოხვდეთ კადრში. თუ გადაღებასთან დაკავშირებით გაქვთ საწინააღმდეგო პოზიცია, გთხოვთ, წინასწარ გვაცნობოთ წერილობით ელ. ფოსტაზე: georgiapcr2018@gmail.com.

Registration Terms and Information Letter

4th International Congress of the Georgian Association of Interventional Cardiologists (GAIC)

By registering, you automatically agree to the terms and conditions outlined below:

1. Basic Registration Rules and Attendance

  • Mandatory Prerequisite: Prior online registration is mandatory to attend the congress. The number of attendees is limited to a maximum of 350 participants.

  • On-Site Registration: During the congress, on-site registration and participant check-in will take place on June 26 from 12:00 to 13:00, as well as on June 27.

  • No-Show Policy: In the event of a no-show or failure to attend the congress, credits will not be issued, and the paid fee is non-refundable.

  • Target Audience: Attendance is intended for cardiologists, interventional cardiologists, residents in relevant fields, and interested medical students. If you do not belong to these categories, please contact the organizing committee before registering (Tel: 599 50 71 87, Email: georgiapcr2018@gmail.com).

2. Credits (10 CME) and Certificate

There are two types of registration for the congress: Professional (with credits) and Non-credit. To receive credits and a professional certificate, the following rules apply:

  • Accurate Data: During registration, it is crucial to provide your exact email address and Personal Identification Number. If requesting credits, all fields must be filled out without errors.

  • Mandatory Attendance: For professional registration and to receive credits, it is mandatory to attend both days of the congress.

  • Evaluation Form: To receive your certificate and credits, you must complete a feedback (evaluation) questionnaire. You will receive this via email, or you can complete it on-site by scanning a barcode.

  • Certificate Issuance: Certificates will be issued exclusively in electronic format and sent to the email address provided during registration.

  • Eligibility for Credits: CME credits can only be obtained by cardiologists, cardiac surgeons, interventional cardiologists, or residents in relevant sub-specialties.

3. Organizational Details

  • Lunch: The dining area will be located on the third floor, in the lunch symposium rooms. Attendees will be provided with lunch boxes, which can be selected on-site during the registration process.

  • Photo/Video Recording: Photography and video recording will take place throughout the congress. Consequently, you may inadvertently appear in the frame. If you object to being recorded, please notify us in advance in writing at: georgiapcr2018@gmail.com.
